Shaded indicators are used ONLY for nutrition assessment. All of the other indicators are used for both assessment and monitoring and evaluation. Each term is designated with an alpha-numeric NCPT hierarchical code, followed by a five-digit (e.g., 99999) Academy SNOMED CT/LOINC unique identifier (ANDUID).
